Summary The Gladney Home Case Manager plays a vital role in delivering clinical case management services that support the overall goals and mission of the Gladney Home program. Description The Gladney Home Case Manager plays a vital role in delivering clinical case management services that support the overall goals and mission of the Gladney Home program. Utilizing a holistic, person-centered approach, the Case Manager works to assess and address each resident’s unique needs by developing, implementing, and monitoring individualized case management plans. Key responsibilities include maintaining accurate case files, creating customized adoption readiness plans, and accompanying residents to medical, behavioral health, and specialist appointments. The Case Manager also supports Gladney Home staff in managing day-to-day resident interactions and serves as the primary liaison for DFPS/SSCC teams, licensing entities, educational institutions, and Youth for Tomorrow (YFT). Additionally, this role coordinates ongoing assessments and ensures access to necessary services and resources throughout the year. Job Duties And Essential Functions Collect information, identifies individual needs, and develops a comprehensive case management plan. Develop trusting relationships with Gladney Home staff. Counsel and guide residents, addressing behavioral concerns when necessary. Develop and utilize all agency and community resources available at the federal, state and local levels. Arrange psychological, CANS assessments, and assessments for psychiatric treatment for residents. Manage resident’s schedules for counseling, equine therapy, and physical and occupational therapy. Act as the resident’s educational decision maker and is also responsible for enrolling the residents in school and attending any school conferences. Solve problems and adapt well to changing needs. Local travel is required and may account for 30% of the time for 5-10 appointments per week depending on the number of residents. Document correspondence and monthly check ins with the residents, document any correspondence with providers, DFPS, or SSCC teams and time sp ent with clients in the case file and case management systems. Work with the residents and internal departments to prepare residents’ adoption readiness plan. Maintain ca se file records and closes files in a timely manner upon discharge. Serve as an example of best practices for Gladney staff in home-based settings. Maintain client records, coordinating the collection of necessary information ensuring compliance with agency policy and regulatory agencies. Maintain a thorough understanding of regulations, requirements, and compliance and is the department contact for audits. Provide and documents aftercare services to residents who have been discharged from Gladney Home. Participate in an on-call rotation during weekends and holidays to provide staff support and respond to crisis situations as needed. Perform other duties and special projects assigned to support organizational objectives and enhance operational efficiency. Qualifications MINIMUM REQUIREMENTS: Bachelor’s degree in social work or related field is required. 2 years of experience with case management. Valid/current driver’s license. Must be able to drive at night. Proficiency in Microsoft Office Word, PowerPoint, and Excel; Microsoft Teams. Must be able to work a flexible schedule due to clients’ needs including weekends, evenings, and nights. Must be energized, motivated, and adaptable to meet changing needs. Superb written and verbal communications skills. Good organizational and time management skills. Ability to deal with defiant behaviors, not easily offended. Must be physically able to carry and lift 30lb and perform de-escalation interventions. Preferred Requirements Experience in client assessment and adoption counseling. Experience with TBRI.
